They actively engage in conducting groups and ensure seamless communication between the treatment team, families, guardians, and community providers. Additionally, the Social Worker takes charge of coordinating all facets of patient discharges, contributing to the overall well-being and successful transition of individuals within the healthcare system.Key Responsibilities

Provide comprehensive therapeutic services and generate biopsychosocial summaries, implementing interventions to address discharge barriers and coordinating all discharge planning and documentation.

Advocate for patients by negotiating internal and external systems within the scope of clinical training, updating discharge lists, and documenting all contacts into the Medical Record.

Enhance patients' capacity to address their needs through therapeutic interventions during all patient contacts.

Collaborate with colleagues to contribute to decisions affecting patient well-being, actively participating in department meetings and sharing ideas for problem-solving.

Demonstrate sensitivity to cultural diversity and individualized care, considering patients' cultures and age-specific developmental needs, while also contributing to the improvement of the facility's service delivery.

Qualifications & RequirementsEducation

Bachelor's degree with three years’ experience in social work or a Master’s degree in Social Work or Mental Health Counseling from an accredited college or university is required. Licensed Clinical Social Worker is preferred.

Experience

Knowledge of the DSM, the chronically mentally ill, and community resources.

Above average interpersonal skills. Must be able to communicate effectively with all levels within the facility, in addition to a variety of outside customers with varying interests
