POSITION SUMMARYThis position provides all the functions of a clinical pharmacist as part of a structured training program to develop the resident’s skills and knowledge as it relates to the practice of clinical pharmacy. Residents are responsible for providing patient-centered, evidence-based pharmaceutical care for patients of ProMedica Toledo Hospital,Russell J. Ebeid Children’s Hospital, and ProMedica Flower Hospital with oversight by clinical staff pharmacists and clinical pharmacy specialists. This position is filled via the American Society of Health-System Pharmacists (ASHP ) recruitment process dictated by accreditation standards, including use of the National Matching Service. ACCOUNTABILITIES

Provides pharmaceutical care for patients per assigned service, including evaluating medication therapy, such as appropriateness of drug selection, drug-drug and drug-disease interactions, and potential adverse reactions. Provision of care occurs with oversight from a clinical pharmacist or clinical pharmacy specialist. The resident is accountable for any additional patient care activities assigned by the preceptor. (Direct patient care – 75%)

Appropriately utilizes the medication use process for preparing and dispensing medications following medication use policies specific to the health system and institution and all laws, regulations, and standards applicable to pharmacy practice. This staffing schedule is set by the residency program director in collaboration with the director of pharmacy. (Direct patient care – 75%)

Identifies and engages in organizational and department quality improvement activities to improve patient care, the medication use process, and pharmacy operations. This may include, but is not limited to, completing a medication use evaluation (MUE), creating or updating a drug monograph for institution-level or system-level Pharmacy and Therapeutics Committee (P&T), and creating and delivering multidisciplinary education. (Scholarly Activity/Service – 25%)

Educates and trains residents, students, interns, colleagues and other health care professionals in a variety of forums, including informal teaching on rounds, formal presentations, and in-service activities. (Scholarly Activity/Service – 25%)

Assesses appropriate drug information and literature resources, and provides effective information to varied audiences including patients, other health care professions, and peers. (Service – 10%)

Designs, develops, and delivers one major project to be presented at a regional conference as determined by the residency program director and residency advisory committee. (Scholarly Activity/Service – 25%)

Meets all the program requirements of a PGY1 pharmacy resident as outlined by ASHP accreditation standards and in the residency program manual.

Performs other duties as assigned.
