The LIFE programs offer an array of person-centered services from medical care to adult day supports with the primary goal of allowing individuals to remain in their own home. Under the supervision of the Director, Clinical & Home Care Services and working closely with the Interdisciplinary Team (IDT), the Home Care RN provides the technical, organizational, and communication skills necessary to efficiently assess the need for and coordinate delivery of home care and clinic services. The Home Care RN must demonstrate the knowledge and skills necessary to assess, plan care for and provide service to frail elders according to assigned responsibilities and LIFE-Program standards. Qualifications Graduated from an accredited school of Nursing and must possess a current Registered Nurse license (RN), in good standing, from the Commonwealth of Pennsylvania. Minimum of one (1) years\' experience working with a frail or elderly population. Working knowledge of computers, Microsoft Word and Excel required.
