We have been helping families stay at home together for over 30 years providing highly trained nurses up to 24 hours a day. Reports to: Administrator Primary Responsibilities: - Responsible for providing physical therapy to Home Care patients and case management when indicated. - Responsible for timely and accurate communication and documentation relative to physical therapy services. - Promotes and develops physical therapy services in homecare/hospice. Specific Job Duties - includes, but not limited to: - Evaluate and treat patients suffering from physical disability due to injury, disease, or surgery in a home care environment. - Perform initial start of care assessment and all related forms. - Develop, revise and, execute an appropriate care plan for restorative and maintenance patients. - Collaborate with physicians, case managers, nurses, home health aides, and office staff to ensure consistent levels of care. - Monitor patient progress and document accordingly. - Educate patients, families, and caregivers regarding the proper use of medical equipment or adaptive equipment. Knowledge of physical therapy principles, interpersonal dynamics, community resources, and legal limitations within the practice of physical therapy. Attends on-the-job and in-service training.
